Nutrition during Pregnancy and Early Chidhood.


The following pargraphs are part of a chapter that I've reviewed with Dr Katz  for the last edition of the Enciclopedya of Primary Prevention.  It was super interesting because I 've learnt many evidence based information about nutrition during Pregnancy and Early Childhood. The Chapater is meant for health professionals particularly, so I admit that it can be a little bit boring and relatively long.  In the next few weeks I will try to generate some more easy-to -read posts   based on this for all my friends who are facing motherhood. 


"In general, pregnancy requires a calorie increase over baseline of approximately 300 kcal/day, and lactation requires 500 kcal/day.  Nutrients for which the recommended daily allowance (RDA) is specifically raised in pregnancy include: total protein, total energy, magnesium, iodine, zinc, selenium, vitamin E, vitamin C, thiamin, niacin, iron, calcium, and folate.  Lactation requires further increases in protein, zinc, vitamin A, vitamin E, vitamin, C, and niacin.  Requirements for iron and folate actually decline   These adaptations in the maternal diet are necessary to assure optimal nutrition for the fetus/newborn.
Immediately postpartum for a period of approximately 3-5 days, human mammary glands produce colostrum, a fluid rich in sodium, chloride, and immune globulins that confer passive immunity to the newborn.  Colostrum is replaced by milk, which is rich in lactose and protein, and comparatively low in sodium and chloride.  Milk volume consumed by the neonate is 50 cc per day at birth, 500 cc by day 5, and 750 cc at 3 months.  Milk production is maintained by infant suckling.  The first four months of lactation consume, and convey to the infant, a comparable amount of energy as the entire gestational period.  Human milk is appropriate as the sole source of infant nutrition for up to 6 months provided it is free of dangerous contaminants or pathogens (e.g., the HIV virus).  There is uncertainty whether milk meets all of the infant's nutritional needs beyond this point.  Breastfeeding, under most circumstances, is the preferred nutritional source for neonates.  A generous intake of dietary calcium, and continued use of prenatal vitamins, is indicated throughout the period of lactation. 
Breast milk and infant formulas differ substantially in a variety of nutrients.  The significance of all of the differences has yet to be established.  The prevailing view is that breast milk favors optimal brain development, and breast-feeding is associated with greater intelligence, at least during childhood, although such observations are subject to confounding.  There is interest in the role breast-feeding may play in preventing the development of atopy (allergy). Evidence is convincing that breast-feeding confers protection against infections, although the mechanisms by which breast milk influences infant immunity remain under study.  Provided that a sanitary water supply is available, the safety of formula is generally not of concern.  Soy-based formulas are available for infants intolerant of bovine milk protein.  Properly nourished, the healthy infant should double in weight by 4-5 months, and triple in weight by 12 months.  Demand feeding is the preferred method of assuring adequate energy intake......"
Strategies That Work
Optimal development in utero is associated with several nutrients in particular.  Supplementation with approximately 400 mg of folic acid per day beginning prior to conception markedly reduces the risk of neural tube defects, including anencephaly and spina bifida.  Evidence for this association has been extensively reviewed and is considered definitive.  Ingestion of more than 1 mg per day of folate is generally not recommended.  However, in women with prior pregnancies leading to neural tube defects, the ingestion of up to 4 mg per day of folate may confer additional benefit.

Practicing Yoga with Music.


I love practicing Yoga just listening to the sounds of my  breath and focusing on what's going in  my mind  in the different Asana, but sometimes it's wonderful just  to let the body flow with some great music. It can be classical, pure rock or kind of new age.

This one I've found is great. I hope you will enjoy it.



Very simple rythm, very primitive. You need to create the melody and armony with your own  movements, listening to the inner sounds. Improvisation.... Trust your body and let it happen.

Working out...Spinning in New York.




Non sono mai stata una grande appassionata di spinning, ma quando Suzanne mi suggerisce di provare qualcosa so che lei non si sbaglia mai.

Ed eccomi a Soul Cycle New York. Una palestra di solo spinning. Una palestra per fissati.

Ambiente super moderno, design divertente. Appena entro mi danno subito un paio di scarpe e mi assegnano la bicicletta. Entro nella stanza , tutti stanno gia' pedalando . Meglio non fare commenti sull'istruttore....




Ed ecco che parte la musica. Mi sembra di essere in una discoteca.  Tutti sono gasatissimi per questo full immersion workout.

3, 2,1 GO!

45 minuti no stop. Allenamento super intenso. Gambe , braccia, resistenza, velocita'....
Marvin , il maestro non perde un colpo. Piu' che un ciclista sembra stia ballando sulla bicicletta.

Quando l'allenamento finisce non mi rendo neanche conto che sono passati 50 minuti. Tutti col sorriso sulle labbra smontano dalle biciclette , pronti per iniziare la loro giornata.

Sara' stata la musica, sara' stato Marvin e l'atmosfera , ma di sicuro la mia opinione sullo spinning e' cambiata. Sebbene sia promotrice delle attivita' all'aria aperta,  per chi ha voglia di allenarsi in maniera intensa, senza sprecare neanche un minuto,  perdere peso e magari ha qualche problema alle ginocchia  questo tipo di work out e' il top. L'endorfina e' assicurata.

Something about Kindness...




Yesterday while reading the book "The Mind's Own Physician: A scientific dialogue with The Dalai Lama on  the Healing Power of Meditation", I've discovered the poem "Kindness" by by Naomi Shihab Nye. I would love to write a lot about this wonderful attitude, but I think the poem speaks for itself. 


Kindness


Before you know what kindness really is
you must lose things,
feel the future dissolve in a moment
like salt in a weakened broth.
What you held in your hand,
what you counted and carefully saved,
all this must go so you know
how desolate the landscape can be
between the regions of kindness.

Uno dei problemi maggiri nei paesi in via di sviluppo e' la mancanza di tecnologia, la ristrettezza di risorse per poter comprare e mantenere strumentazione e macchinari che nei nostri paesi vengono utilizzati quotidianamente.
La sonda ecografica con la relativa application per smart phone rapppresenta un avanzamento tecnologico che puo' davvero fare la differenza. Facile da usare, maneggevole e puo' essere ricaricato con estrema facilita' senza bisogno di tenerlo sempre collegato alla corrente. Voto  10!

Uno dei migliori video che abbia mai visto sullo sviluppo del feto dal concepimento alla nascita. Penso che non servano altre parole. Il miracolo della tecnologia per spiegare il miracolo della vita.





Hypnobirthing: usare la mente durante il parto.



Paese che vai usanza che trovi. E adesso per il parto e la gravidanza  ce n'e' davvero per tutti i gusti. Se gia' qualcuno pensava che parto in acqua e liane fossero all'ultimo grido,  cosa dire dell hypnobirthing ?

Una sorta di ipnosi, di training autogeno per prepararsi al parto e viverlo nella maniera piu' tranquilla e naturale possibile.
Le adepte dell'hypnobirthing sono numerosissime in Ingilterra e in tutti paesi Anglosassoni.
Chi l'ha fatto sembra essere davvero entusiasta. Trovare pace nella mente e nel corpo perche' il parto avvenga senza sforzo, e secondo molte, con una notevole riduzione del dolore.
Ieri ho ascoltato solo qualche minuto di un MP3 di Hypnobirthing che un' amica sta utilizzando  per prepararsi al parto : " You are the natural vehicle...." Solo la voce portava ad un 'altra dimensione.
La sequenza va ascoltata il piu' possibile  cosi' da sapere automaticamente cosa fare e cosa VISUALIZZARE  nel momento del bisogno e del dolore. E se il posto dove vi piacerebbe partorire e' una bella spiaggia alla Maldive alle ore del tramonto non c'e' niente di male.

La tecnica e' stata sviluppata da un ipnoterapista nel 1989 , M.Morgan sulla scia di un libro del Dr. Dock-Read sugli effetti del dolore durante il parto.  La teoria ritiene che il 95% del dolore sia attribuilbile alla tensione e alla paura.
 Un interessante forma di mind body medicine. Un training che viene fatto sulla mente che permette di avere un nuova percezione del parto, senza pero' togliere la consapevolezza del momento. La madre non e' ne' in uno stato di trans ne' dormiente.

Adesso se vedete entrare una mamma con cuffiette e Ipod in sala parto sapete perche ;-)!
